Frequently Asked Questions:

Can I use my insurance?

  • Our monthly fee is not covered by insurance, and we will not bill insurance for our providers’ care. That said, you may be able to use a FSA or HSA to pay your membership fee. We can use insurance for other healthcare needs, such as additional needed labs, imaging, referrals, etc.

What is included in the cost?

  • All visits: in-person, phone calls, e-mails, texts
  • Annual labs: We include standard annual lab testing as a part of our membership fee. If we need to collect labs more than once per year, or want to collect more labs than we would typically do as a part of an annual visit, we will ask you if you want to use our low-cost lab fees or if you want us to submit the labs for payment through insurance. Either way, we can collect the labs right here in our office!
  • Joint injections
  • Minor skin procedures
  • Reduced price wholesale pharmacy, on-site! This gives patients access to huge potential savings and the convenience of avoiding the retail pharmacies. Of note, we do not stock any controlled substances.

What if I go to the hospital?

  • We do not have an affiliation with any single hospital system, and would recommend choosing a hospital based on your specific health need and urgency of your situation. We do not write orders for our patients in the hospital or go into the hospital, but will coordinate with the admitting doctor as needed to make sure you are getting the best care and that we have a good plan after discharge. What if I get sick?

  • If you get sick or think you may need to be seen for another concern, reach out to us by call, text or e-mail. We will then advise you on the best next step and get you in to see us if needed. Same-day or next-day appointments are available. Oftentimes, our physicians can advise about an issue without an office visit, saving you time.

What is your electronic medical record system? How do you coordinate with other doctors?

  • Our electronic medical record system allows us to communicate with you, coordinate your care amongst our team, electronically prescribe medications, and document your care appropriately. There is a patient portal, though it is not necessary for you to use it and most patients do not.
  • While we don’t use the same medical record system as the big health systems here in Louisville, we do have access to their record systems so that we can review past records, coordinate with specialists, and order testing.
  • We use LabCorp to process labs done in our office. LabCorp sends us a pdf report of all lab results, which we share with our patients. Patients can also access all of their labs, and view their lab trends, by logging into LabCorp’s patient website.

What do I do if I have a question about my health?

  • You can always reach your doctor (or physician assistant), directly for any questions you have about your health or your care. You can email, call or text with any questions or concerns.

How do I sign up?

  • We recommend you fill out the online enrollment form as a first step. To do this, click “Enroll” and follow the instructions. After you’ve submitted the online form, we will review and process your enrollment and reach out to you to set up a first appointment. What if I want to learn more?

  • We are happy to do a “meet & greet” with you so that you may get to know a provider, see the practice and ask any questions you may have. These are typically short 15-30 minute visits and do not include health assessments or health recommendations. Call us at our main office number to schedule: 502-805-6448.

What if I decide to cancel my membership?

  • We do not ask patients to commit to any long-term agreement with our practice. If you decide you want to unenroll, just let us know and we will stop any future billing. You will not be refunded for previously billed months.

ABOUT DIRECT PRIMARY CARE – HOW IT WORKS

MEET ILANA KAYROUZ, MD                                                           

Dr. Ilana Kayrouz is a board-certified physician in both Internal Medicine by the ABIM and Pediatrics by the ABP.  She graduated from the University of Louisville School of Medicine in 2006 and completed her residency in combined Internal Medicine and Pediatrics in 2010 in Louisville as well.

She was born and raised in Louisville and has practiced medicine in Oldham County, just outside of Louisville since 2010. Dr Kayrouz is married to Ron and between them have five children and six grandchildren.

Her passions outside of medicine and her family are travel, culinary arts, and photography. She loves to experience the beauty and culture of this world and capture it on camera. She enjoys creating new recipes, especially when she can share them with others. She also values fitness and health and regularly walks, hikes, swims, bikes and participates in TRX and other strength training. 

Dr Kayrouz was voted best physician in Oldham County in 2023! She is a strong advocate for the direct primary care model of healthcare as she feels she can truly care for patients in the way she dreamed of. Her patients are her priority and she is proud to offer them the best care. 

MEET AMBER DEPTOLA, MD                                                                       

A first generation physician, Dr. Amber (Zimmer) Deptola decided to pursue a career in medicine after volunteering at Norton Children’s Hospital in high school. During her volunteer experiences, she realized the great potential for physicians to connect with their patients and partner with them towards better health, an aim she continues to strive for in her care now as physician.

Originally from Ohio, Dr. Deptola completed her undergraduate degree at Miami University in Ohio and then returned back to Kentucky for her medical school training at the University of Louisville, where she graduated with highest honors. After she completed her residency training in Internal Medicine at Washington University in St. Louis, Dr. Deptola served as a Chief Resident, and then remained there as a faculty member for 8 years. As faculty, Dr. Deptola cared for patients, taught, and directed multiple programs for medical students and resident physicians. In addition to teaching evidence-based medicine, Dr. Deptola trained resident physicians on communication skills, including the use of coaching and motivational interviewing in patient care.

She believes strongly that building close relationships with patients, and use of these communication strategies, is integral to helping achieve health goals that are personalized to each patient. Dr. Deptola returned to Louisville with her husband and 2 children in 2022 and is looking forward to using her training and experiences to care for patients at 502 Direct Primary Care.

MEET LEXI MCCLANAHAN, PA-C

Lexi McClanahan is a board-certified physician assistant. She graduated from Sullivan University June of 2023. Originally from Ohio, she has grown up visiting her relatives in Oldham County and finds it home to some of her most precious memories. She is excited to get to practice medicine in such a familiar, special place!
Lexi is married to Elijah McClanahan, in their free time they enjoy reading & looking for new books to add to their growing library, outings with their mini-Australian shepherd, trying out local restaurants and attending church events.
Additionally, Lexi enjoys crafting, teaching Sunday school to first graders at her local church and serving on her church’s medical response team.
She looks forward to establishing collaborative relationships with her patients and aims to give them all the tools and resources necessary to improve or maintain their health and wellbeing.

MEET STEPHANIE BURDEN, CMA

Stephanie Burden is a Certified Medical Assistant with the NHA. She has worked as a medical assistant for 13 years, 12 1/2  of them with Dr.Ilana Kayrouz.

She has been married for 18 years to her best friend Bill has one son. She also recently welcomed and daughter in law to the family. They will welcome their first grandchild in May!

She enjoys traveling, anything outdoorsy, ancient history
and trying new things; especially food. Her best past times are spending quality time with her family and friends, reading a great book, and letting her nerdy side out by chilling with her husband and indulging in sci-fi television. 

Stephanie is extremely valued and loved by patients because of her ability to make them feel comfortable and well taken care of. She has enjoyed being able to take more time with patients and have a closer relationship in the direct primary care model.

MEET JENA WALDMAN, CMA

Jena Waldman is a mom of three girls and a medical assistant. She finds great joy in helping and making a positive impact on their lives.

 Whether it’s assisting the medical team during procedures, comforting patients during their visit, or providing crucial help to healthcare professionals, she takes pride in the important role she plays in the well-being of others. The sense of fulfillment that she gets from seeing patients improve and thrive is truly unparalleled.

As a mom, she is constantly amazed at the love and joy that comes with raising a child. Watching them grow and learn every day brings her immense happiness and fulfillment. The bond she shares with here child is unbreakable, and she cherishes every moment they spend together, creating memories that will last a lifetime.

Balancing these two roles allow her to experience the best of both worlds, and she is grateful for the meaningful and rewarding experiences that come with being both a medical assistant and a mom.

She enjoys being outdoors, spending time with family, hiking, shopping, and traveling. She finds immense pleasure in exploring the beauty of nature and creating unforgettable moments with her loved ones.
